Use this form to provide or update your details to Horizon Church to receive information on our services, events and birthday cards.

If you are updating details, you only need to enter your name, email and the information that has changed.

Date of birth is optional but appreciated. If you would like to receive a birthday message without providing your date of birth, type in just your birth day and month with no year (e.g. in 23/12).

For new registrations please add details of your family members, especially children who will attend children's church.

For updating your details, only enter if you are updating the details of your family members.
